The Effect of Posterior Subtenon Triamcinolone Acetonide on Diabetic Macular Edema

Abstract
We evaluated posterior subtenon triamcinolone acetonide injection for diabetic macular edema placing priority to safety, improvement of visual acuity and anatomical effect. Thirty-nine posterior subtenon injections of 40 mg triamcinolone acetonide were performed in 39 eyes of 32 patients and all patients received examination including intraocular pressure (IOP) measurement, best-corrected Snellen visual acuity, macular thickness and types of macular edema. In 39 eyes, cystoid macular edema (21 eyes, 53.8%), sponge like retinal swelling (15 eyes, 38.5%) and serous retinal detachment (3 eyes, 7.7%) were found in optical coherence tomogarphy (OCT). Visual acuity and macular thickness improved at the 3-month evaluation without complications including increased IOP, cataract and endophthalmitis. Improvement in visual acuity and decrease in macular thickness were observed on diabetic macular edema until 3 months after injection but decrease in visual acuity and increase in macular thickness were observed after 6 months. Though this result is similar to that of intravitreal triamcinolone acetonide injection, posterior subtenon injection of triamcinolone acetonide is thought to be the better treatment in diabetic macular edema due to no complication.
